What are the rules which must be applied for unusual types of numbering : 5 bis or 5 ter, 4 to 8, 12/14 ?

5 bis or 5 ter is shortened to 5 B or 5 T. In the event of a number with 2 separate sets of figures, only the first number should be indicated in line 4 (4 to 8 should be written as 4 and 12/14 should be written as 12).
